VERY IMPORTANT - PLEASE READ BEFORE BOOKING: Every participant in school sports or extracurricular activity is required to complete a pre-participation physical examination, also called sports physicals. DOTexamclinic.com offers sports, school athletic and camp physicals for all levels of students. If you have more than one child, each child will require an individual/separate appointment. If a child is a minor (under the age of 18), a legal guardian (not underage sibling or neighbor) MUST be present and remain at the facility throughout the exam. IF YOU ARE A COLLGE/UNIVERSITY STUDENT, ENSURE THAT YOUR FORM DOES NOT SPECIFY PHYSICIAN (MD/DO) ONLY. ADDITIONALLY, IF YOUR EXAM REQUIRES A MENTAL HEALTH/PYSCHOLOGICAL EVALUATION, YOU WILL NEED TO SEEK OUT ANOTHER MEDICAL FACILITY FOR YOUR EXAM.
